Trinity Transition Housing provides safe, stable shared housing for individuals who are receiving support through licensed professionals and social service organizations.

We work with individuals living with:

  • Other life circumstances requiring stable housing

We do not provide medical care, therapy, case management, or behavioral health services.

All services and support are coordinated and delivered by each resident’s assigned care team, case manager, or partner agency.

Our homes are designed to provide:

We do not provide medical care, clinical services, or case management.

All services and support are coordinated and delivered by the veteran’s assigned VA team, case manager, or partner organization.

We collaborate with organizations that serve veterans to help ensure individuals have access to safe and reliable housing while continuing to receive the care and support they need.

We do not provide medical care, personal care, or case management services.

All support services are coordinated and delivered by each resident’s assigned care provider, case manager, or partner organization when applicable.

Trinity Transition Housing provides safe, stable shared housing in a substance-free living environment for individuals in recovery who are working with support programs or recovery-based organizations.

Our homes offer a structured and respectful setting that supports individuals committed to maintaining sobriety while continuing their recovery journey.

We do not provide addiction treatment, counseling, or clinical services.

All recovery support services, including counseling, meetings, and case management, are coordinated and delivered by each resident’s sponsor, program, or partner agency.

Our sober living homes are designed to provide:

  • Drug- and alcohol-free housing environments
  • Structured and respectful shared living
  • Stability for individuals actively engaged in recovery programs

Residents are expected to maintain a substance-free lifestyle while residing in the home.
