Group Care

Our homes are located in safe, established neighborhoods with convenient access to public transportation, healthcare facilities, and community resources. Each property is designed to provide a clean, stable, and respectful shared living environment that supports individuals in transition. Trinity Transition Housing provides housing only. Our homes are not medical or treatment facilities, and we do not provide healthcare, clinical services, or case management.

Your care, your needs

Residents may receive services through their assigned providers or partner organizations. These services are coordinated independently of Trinity Transition Housing and may include:

  • Medical care and home health services
  • Therapy and behavioral health support
  • Case management and social work services
  • Medication management
  • Hospice or specialized care when applicable
  • Durable medical equipment support

All services are arranged and delivered by licensed third-party providers, case managers, or healthcare organizations.

Our homes are part of the surrounding community and are intended to provide a stable housing foundation while residents continue to engage with their external support systems.
